Interprofessional Primary Care Team
Temporary Part‑Time, Day‑Evening Shift

Duration: 5 months with possible extension
Shift Type: Day‑Evening, some weekends (subject to change)
Hourly Pay Rate: $40.24 - $57.68

Job Summary:
Don’t miss this opportunity to be part of Baycrest’s Ambulatory Services. Through this role you will work as a member of interprofessional primary care outreach teams and the Neighbourhood Care Team. You will provide health navigation, nursing clinics and chronic disease management for clients ages 18+, living independently in the community Toronto Senior Housing Buildings, who can benefit from an interprofessional team approach to address physical, cognitive, psychosocial, social and environmental issues that are having an impact on function, with the focus on chronic disease management, frailty, mental health and primary care. You will provide health education to help clients achieve their health goals to improve their quality of life. Your assessment and goal setting will inform the comprehensive assessment and treatment plan of clients. In this role you will be working closely with primary care physicians to support attachment to primary care and collaborative care. You will also work with community partners and interprofessional teams on coordinated care.

Responsibilities include but are not limited to:
- Provides sound clinical knowledge and skills, acts as clinical expert and resource and provides comprehensive, quality client/family centred health care services from admission, through transfers and discharge
- Help develop and deliver health education and health promotion activities in the community.
- Supports the development of Chronic Disease care plans and provides coaching and client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of a clients
- Liaises with housing and other community providers to work as one team in caring for mutual clients. Participates in team meetings and contributes to team development
- Participates in data collection and reporting on a number of accountability measures
- Performs teaching responsibilities, participates in relevant education and research activities, and performs duties consistent with the job classification, as required.
- Performs departmental responsibilities to contribute to the efficient and effective provision of service.
- Ensures the protection of client rights and acting in an advocacy role for the resident/clients.
Qualifications include but are not limited to:
- Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required
- Four (4) year B.Sc. Nursing Degree required
- Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C – Automated External Device (‘AED’) is required
- Access to a reliable transportation is required due to regular travels in the community
- Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for clients, families and care teams.
- One (1) year recent related experience in geriatrics, community based seniors care and behavioural management with expert practice skills; 3 years preferred.
- Advanced nursing competencies relevant to clinical area of practice.
- Previous primary care experience preferred
- Experience working in the Toronto Senior Housing and primary care sectors preferred
- Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours including various types of dementia, delirium, mental health issues preferred
- Knowledgeable in community services
- Knowledge of infection control measures and bed bug preventative practices in the community preferred.
- Ability to travel to different locations
- Excellent interpersonal, communication and problem-solving skills
- Basic computer skills – Meditech, MDS/RAI, Microsoft Word, Teams, Zoom and Excel preferred
