<p>This opportunity is with our Good Shepherd – HOMES Program</p>
<h3>How You Will Make an Impact</h3>
<p>The HOMES program provides supportive housing to persons who have been homeless and are living with a mental illness. Team members support tenants in scattered apartment units throughout the city to reach personal goals and to have a satisfying and meaningful life in the community.</p>
<ul>
<li>Responsible for assessment, planning, crisis prevention and intervention, supportive counselling, education, advocacy, conflict resolution between tenants and landlords</li>
<li>Facilitating social interaction and community integration</li>
<li>Provide linkages with other community services</li>
<li>Provide primary support to individuals and support their recovery planning</li>
<li>Some opportunities include work done in the community and in tenant apartments, with constant driving required for Mobile Team</li>
<li>Observe all health and safety rules and work in compliance with the provisions of the occupational Health and Safety Act and its regulations</li>
<li> ауыр be willing and able to assist in a hands‑on capacity with tenants</li>
</ul>
<h3>What You'll Bring to Our Team
<ul>
<li>Primary case management and community development skills acquired through a post‑secondary education in social work, registered practical nurse or other human service field</li>
<li>Minimum of five years’ experience in mental health in a community‑based setting</li>
<li>Worked in an outreachтиқ mobile atmosphere is an asset</li>
<li>Knowledge (*.c) of community resources, the forensic system and the social and environmental determinants of health</li>
<li>Demonstrated cultural sensitivity</li>
<li>Must have recovery orientation and consumer‑driven style of practice</li>
<li>This position requires a valid driver's licence (for our Mobile Team/CHO Team) and must be willing to use a personal vehicle at work and acquire appropriate insurance</li>
<li>Time management skills are necessary in a high‑energy, changing environment</li>
<li>Individual and team problem‑solving capability is essential</li>
<li>Pro‑active mañana approach to service provision</li>
<li>Committed to anti‑racist/anti‑oppressive practice and to the mission and values of Good Shepherd</li>
<li>Fluency in a language in addition to English is an asset</li>
</ul>
